Occupational Health for Employees
The Village of Rosemont has selected a new occupational health service to send our employees who are injured on duty.
Although we cannot make people go to any one location we would like to encourage them to go to Advocate Medical Group as it will be easier to track and follow up with these injuries. We spent some time looking at different facilities and we were very impressed with Advocate Health. The closest location is about 15 minutes away in Elk Grove on Elmhurst road but they have 13 other locations throughout the Chicago land area and any after hour injures would go to Lutheran General ER. Additionally, follow up therapy would be through Lutheran General giving people a vast selection of doctors to choose from. We are also going to have them run our random pool for all our CDL and bus drivers. If an employee is injured, their supervisor will need to sign the “Authorized Signature” line and fill out the “Client Information” and “Type of Service Requested” on the Authorization for Treatment Form.
The employee will need to fill out a Form 45, the authorization for treatment is only for the initial doctor visit so they are streamlined and attached to the Village of Rosemont. Obviously, if an employee is seriously injured, they should be transported to the ER in an ambulance. Advocate is able to handle cuts, abrasions, stitches, sprains etc.
