Occupational Health for Employees
The Village of Rosemont now has three occupational health services for our employees who are injured on duty.
The Village of Rosemont now has three Occupational Health Centers for our employees – Advocate Medical Group -- located less than 15 minutes away on Elmhurst Road in Elk Grove, Concentra located on Jarvis Avenue in Elk Grove and Physicians Immediate Care located on Cumberland Road in Norridge. They all have very progressive clinics that are available for all on-duty injuries. Additionally, Physicians and Concentra are Urgent Care Centers that take anyone who is in need of a walk-in clinic. Advocate has a total of 13 locations and is affiliated with Lutheran General Hospital. Physicians Immediate Care has a total of 36 locations, 6 located within ½ hour of Rosemont. Concentra has 16 locations, 5 located within a ½ hour of Rosemont. Additionally, if an employee is injured on duty they will need to bring an authorization form signed by their supervisor to the clinic. These forms are available on-our website or through Advocate’s website. Obviously, if there is a life threatening injury the employee should call 911 and go to the nearest hospital.
The employee will eventually need to fill out a Form 45 with their direct supervisor, the authorization for treatment is only for the initial doctor visit so we can better track and handle your workman’s compensation issues.
